One of the primary goals of this comprehensive effort to improve health among African-American families, mothers and babies is to increase support and use of systems and services for chronic disease prevention through clinical-community linkages. We recognize that we can only achieve this goal through partnership and collaboration with organizations.
We have created this brief form to enable you to refer the women you serve to essential health services, including Baby Cafes, the Baby and Me Tobacco Free program, and food pantries. This document explains the referral process.
When you complete the form, you will receive a confirmation email and the person you are referring will receive a notification email letting them know someone from the Gulf Coast Healthy Families, Mothers, and Babies team will contact them to connect them to the services they need.
